Following are the answers to the 3 cases found in the Coronary CTA Revolutionizing Cardiac Care brochure.

Case 1

Coronary Artery Anomaly

40 year old with family history of heart disease. Diagnosis?

Answer:

ALCAPA = Anomalous origin of left coronary artery off PA.

Incidence of ALCAPA is 1/300 000 live births (between 0.24% and 0.46% of all congenital cardiac anomalies).   Usually presents in first year of life with CHF, ischemia or failure to thrive.  If not treated surgically, up to 85% of patients with ALCAPA die in infancy or childhood, and untreated ALCAPA in the elderly is extremely rare.  This patient was surgically treated by ligation of the anomalous artery at its origin and a LIMA to LAD graft.

Coronary Artery Stenosis

53 year old male with reversible perfusion defect in the inferior wall. Diagnosis?

Answer:

Severe stenosis in the proximal RCA which was successfully stented.

Case 3

Noncoronary Cardiac and Ancillary Findings

50 year old female with syncope. Diagnosis #1? Diagnosis #2?

Answer:

Diagnosis #1:  Hypertrophic Obstructive Cardiomyopathy - diffusely thickened LV myocardium with left ventricular outflow tract obstruction by the anterior leaflet of the mitral valve.

Diagnosis #2:  Right breast cancer with internal mammary lymphadenopathy
